Graduation Application Form
As a graduate student, you are responsible for verifying that you have met all degree requirements by the deadlines printed in the Graduate Bulletin. If you do not meet the various deadlines, you will not be able to graduate.
When you have met or will meet all degree requirements by the posted deadlines, submit this Graduation Application to the Graduate School of Management by the deadline listed in the Graduate Bulletin. Until all graduation requirements have been met, you must maintain the appropriate registration status (less than half-time, half-time, full-time).
If you omit required information, this form will not be processed by the Graduate School of Management.
Please note that your name appears in the graduation program for the first graduation date you submit on this form.
- Students who apply for May or August graduation — but defer and reapply for December graduation — will see their names in the May graduation program.
- Likewise, students who apply for December graduation — but defer and reapply for May graduation — will see their names in the December graduation program.
